Вагінізм: частота, визначення, діагностичні критерії, формування

Анотація
У статті наводяться частота вагінізму, його визначення і критерії, які надаються в енциклопедичних джерелах, Міжнародній класифікації хвороб 10-го і 11-го переглядів (МКХ-10 і МКХ-11), Діагностичному та статистичному посібнику з психічних розладів 5-го перегляду (США) (ДСР-5), а також у публікаціях деяких авторів. Повідомляється, що якщо в МКХ-10 психогенні і біогенні варіанти вагінізму представлені окремо, то в МКХ-11 розмежування сексуальних дисфункцій на психогенні і органічні було скасовано. У МКХ-11 мозок і тіло розглядаються як єдине ціле, при цьому вважається, що сексуальні дисфункції пов’язані із взаємодією фізичних і психологічних факторів. Таким чином, було скасовано відмінність між органічним і неорганічним, а органічні і неорганічні розлади були об’єднані.
У МКХ-11 вагінізм був рекласифікований у сексуальний розлад, що пов’язаний з болем при проникненні [sexual pain-penetration disorder] без поділу його на органічні інеорганічні форми, і має єдиний шифр HA20. Діагноз «диспареунія» в МКХ-11 зберігся, але без поділу її на неорганічні і органічні варіанти. У ДСР-5 використовується термін «генітально-тазовий біль/розлад пенетрації» («genito-pelvic pain/penetration disorder»). У даному випадку мова йде про сукупний діагноз, який поєднує вагінізм і диспареунію. Рішення про доцільність використання такого узагальненого діагнозу в даній класифікації було прийнято з посиланням на той факт, що обидві названі сексуальні дисфункції є у високому ступені коморбідними і важко розрізнюваними. Також у статті названі м’язи, які залучаються до процесу скорочення при вагінізмі, і обговорюється можливість існування у людей феномена полону члена (penis captivus). Крім того, докладно перелічено соціогенні, психогенні і соматогенні фактори, які можуть брати участь у формуванні зазначеної патології.
